You will automatically be charged for your child's dues.  If you would rather pay the dues in installments or $0.50 weekly, please make a note in the "BILLING NOTES" section.

Please enter any allergies and/or special needs of your clubber in the SPECIAL NOTES section.

If your clubber requires a uniform, please check the box and indicate the size at the bottom of the form.

The maximum cost per family for handbooks and shirts is $50. This does not apply to dues which can be paid at the beginning of the club year ($14 per clubber) or weekly (50 cents). Your account will be adjusted by Awana staff after registration if your family exceeds $50 in handbooks and uniforms.
